Cost of Treatment
All of Dr. Meihofer’s services listed below are booked in sessions. Each session may vary by length and focus, as they are guided by your needs and progress.
The prices below reflect the cost of Dr. Meihofer’s services before tax.
In-Person Office and Virtual Visits

8-pack follow-up: $1,560 ($195 per session, save 120 dollars)
12-pack follow-up: $2,160 ($180 per session, save 360 dollars)
18-pack follow-up: $2,970 ($165 per session, save 810 dollars)
*All above services are eligible for submission to insurance unless you are seen virtually and physically outside Minnesota at the time of service. Those telehealth services will not be eligible for submission to insurance and will be billed as a wellness visit.

After-Session Guidance
In order to support your progress between sessions, Dr. Meihofer may provide educational materials, guides, or other forms of communication via a secure patient portal. These materials are provided to help you manage an effective at-home program which may include exercises or other strategies aligned with your treatment plan.

While You Are Under Dr. Meihofer’s Care…
Support Between Visits
Dr. Meihofer understands that her patients may need to contact her outside of their regularly scheduled appointment times—and she is happy to answer questions or be a source of information!
However, some requests require dedicated clinical time, and will therefore be billed at $55 per 15-minute increment. Some examples of requests that require clinical time include the following:
- Requesting letters of medical necessity or other documentation
- Treatment support, including clarification on exercises or other at-home activities
- Specialty projects such as ergonomic assessments, exercise programming, etc.
- Coordination with other providers
Brief questions and other simple communications are addressed without charge. Dr. Meihofer suggests that all patients bring questions to their scheduled visits so they can receive timely guidance. It can also be helpful to take notes during or immediately after your appointment so that you won’t forget any important details!
For All Visits
Dr. Meihofer works in a cash-based practice, meaning full payment is due at the time of service. She is out-of-network with all insurers—however, she accepts HSA/FSA payments, offers discounted treatment packages, and provides superbills that you can submit to your insurance for out-of-network coverage.
When booking an appointment with Dr. Meihofer, a valid credit card will be required at the time of scheduling.
About Treatment and Diagnosis Codes
Dr. Meihofer will select diagnosis and treatment codes based on your evaluation, symptoms, medical history, and the care provided during each session. Diagnosis codes are determined during your evaluation, and treatment codes are selected at each visit based on the services that are medically necessary and performed that day.
Please contact your insurance provider before starting care to verify your out-of-network benefits and coverage details.
These codes cannot be retroactively changed for insurance, reimbursement, or billing preference purposes. We strongly recommend reviewing your benefits ahead of time so you understand your potential financial responsibility before beginning treatment.

- 97161–97163 – PT Evaluation (low/mod/high complexity)
- 97164 – PT Re-evaluation
- 97110 – Therapeutic Exercise
- 97112 – Neuromuscular Re-education
- 97535 – Self Care / Home Management Training
- 97140 – Manual Therapy
- 97530 – Therapeutic Activity
- 97116 – Gait Training
*Cost per treatment code will depend on whether you opt to pay full price for the session or choose a treatment package.

Cancellations and
Missed Appointments Policy
Patients who fail to reschedule or cancel their appointment at least 24 hours prior to their appointment time will be charged in full for the cost of their visit. However, Dr. Meihofer is always able to switch to a video visit or phone visit to accommodate last minute changes.

Refund and Return Policy
Dr. Meihofer does not offer returns or refunds, meaning all sales made on both services and service packages are final. This includes discounted package services.
Service packages or discounted plans are final, non-transferable, and non-refundable. Available visits on your package expire one year after purchase. If you have unused visits at the end of your therapy journey they may be carried forward for continued care, including maintenance visits, to address a new concern, or to reinitiate treatment for prior concerns.
Good Faith Estimate
The No Surprises Act allows patients who do not have or choose not to use insurance for their services (self-pay) the right to receive a Good Faith Estimate (GFE) in writing (either on paper or electronically).
