Cost of Treatment
Important: All of Dr. Meihofer’s services listed below are booked in sessions lasting 60 minutes (other than her movement screen and discovery call). She may be able to adjust the length of your appointment to meet your needs. However, extending your session time will include an incremental upcharge. Additionally, the prices below reflect the cost of Dr. Meihofer’s services before tax.
In-person Office and Virtual Visits
Free Discovery Call: $0/20 minutes
Movement Screen: $70/20 minutes
Initial Evaluation: $210/60 minutes
Follow Up Visit: $210/60 minutes
Re-evaluation: $210/60 minutes
8-pack follow-up: $1,560 ($195 per session, save 120 dollars)
12-pack follow-up: $2,160 ($180 per session, save 360 dollars)
18-pack follow-up: $2,970 ($165 per session, save 810 dollars)
*All above services are eligible for submission to insurance unless you are seen virtually and physically outside Minnesota at the time of service. Those telehealth services will not be eligible for submission to insurance and will be billed as a wellness visit.
In-home Physical Therapy
Free Discovery Call: $0/20 minutes
Evaluation: $210/60 minutes
Follow Up: $210/60 minutes.
Re-evaluation: $210/60 minutes
Additional Time with Patient: $55
(Billed in 15-minute increments)
Door-to-Door Drive Time: $55
Will be calculated and billed in 15-minute increments

Miscellaneous Fees
She understands that her current patients may need her to contact their provider outside of their regularly scheduled appointment times—and she is happy to answer questions or be your source of information! However, with the large number of patients Dr.Meihofer treats, her availability is very limited. Therefore, Dr. Meihofer requires a time-based fee of $3.50 per minute when meeting (in-person or virtually) with a provider or patient for the following reasons:
- Requesting letters of medical necessity
- Treatment support, including clarification on exercises or other at-home activities
- Speciality projects such as ergonomic assessments, exercise programming, etc.
To avoid incurring additional miscellaneous fees, she recommends making preparations ahead of time before your appointments, such as writing down any questions you have, so you don’t forget to ask them during your allotted time! It may also help to take notes immediately after your appointment (while they are fresh in your mind) so you can remember things better later.
For All Visits
Dr. Meihofer works in a cash-based practice, meaning full payment is due at the time of service. She is out-of-network with all insurers—however, she does accept HSA/FSA payments, offer discounted treatment packages, and provide superbills that you can submit to your insurance for out-of-network coverage.
When booking an appointment with Dr. Meihofer, a valid credit card will be required at the time of scheduling.
About Treatment and Diagnosis Codes
When starting treatment with Dr. Meihofer, she will determine diagnosis and treatment codes that accurately reflect your treatment and plan of care. Diagnosis codes are decided at the time of your evaluation, and treatment codes are determined at each session. These are unable to be altered at any point during the remainder of your care. In other words, any billing related to these codes cannot be adjusted after they are entered into our system.
Therefore, please be sure to contact your insurance beforehand to discuss coverage, and have that information prepared ahead of time when booking your evaluation with Dr. Meihofer, in case you have questions about billing. *Cost per treatment code will depend on whether you opt to pay full price for the session or choose a treatment package.

Cancellations and
Missed Appointments Policy
Patients who fail to reschedule or cancel their appointment at least 24 hours prior to their appointment time will be charged in full for the cost of their visit. However, she is always able to switch to a video visit or phone visit to accommodate last minute changes.

Refund and Return Policy
Dr. Meihofer does not offer returns or refunds, meaning all sales made on both services and service packages are final. This includes discounted package services.
Service packages or discounted plans are final, and available visits on your package expire one year after purchase. If you have unused visits at the end of your therapy journey, these visits may be gifted or saved for future use.
Good Faith Estimate
The No Surprises Act allows patients who do not have or choose not to use insurance for their services (self-pay) the right to receive a Good Faith Estimate (GFE) in writing (either on paper or electronically).
